During the crossover episode, when he was shot, he went to the hospital, where the doctors informed him that he might have cancer. Later, he was diagnosed with prostate cancer. Though he initially hid the truth from Moira, he later told her everything.
In Monday’s episode, the character underwent cancer surgery. His operation was successful, but he is still fearful. In the latest episode, Moira came to visit Cain, who was told by the doctor that he would be able to leave the hospital by the end of the day if he was able to walk. When Cain’s wife left, he was anxious about seeing the catheter. He was fitted with it after the operation. Later, Cain talked to the doctor, who told him that he would have the catheter for at least two weeks.

She also added that he might also experience incontinence when the catheter is removed. Then Cain was surprised as he didn’t want to go home with the catheter and keep it in for so long. Meanwhile, when Caleb visited him, he was angry and told him to leave. When Caleb tried to make him happy, Cain shared that he wished he had never had the operation, as he didn’t want to go home in his current state.
Caleb told Cain that he had people to look after him, but Cain said that he didn’t want that. Furthermore, he didn’t want others to know about his catheter. Cain added that he would rather have risked his life than live like this. Later, when Moira arrived to take Cain home, the doctor arrived to see him. He told her that standing up was too painful.
