Though Ruby and Dingles were successful in covering up the murder, the medic recently blackmailed Caleb.
When John’s crimes were out, he decided to flee, so he used Caleb Miligan. He told him to help; otherwise, he would send an email to the police exposing Ruby’s crime. Thus, he had to help the killer protect his wife. When Cain learned about it, he was furious and went to search for John Sugden in France.
